The main focus of the GeReNet.Wi between May 2004 and April 20047 was to improve the situation of the elderly after an in-patients stay in hospital, with the aim of improving discharge and transition and securing living autonomy by intensifying the cooperation at the interface between hospital and out-patient care.
The research assistant performed an as-is analysis in May 2007, which investigated all relevant professions including medicine, care, social work and therapists, both in hospital as well as in the out-patient sector. The nursing homes in Wiesbaden were also involved in the analysis.
In total 21 departments in 4 hospitals took part in the study. The response rate of the clinical staff (doctors, nurses, social workers and therapists) was very satisfactory (75%). Some 263 departments, services and clinical practices were involved in the study.
The results and recommended follow-up actions were published in a final report.
